I UNDERSTAND THAT NO WORK SHALL COMMENCE UNTIL WRITTEN APPROVAL IS RECEIVED. IF WORK IS BEGUN WITHOUT PRIOR WRITTEN APPROVAL, I UNDERSTAND IT IS IN VIOLATION AND I MAY BE ASKED TO REMOVE IT AND BE SUBJECT TO A $100.00 FINE AND $45.00 ARC SUBMITTAL FEE.

All applicable codes and regulations will be followed and all necessary permits will be obtained and included with this request. I have read and understand all applicable sections of the FMHOA Governing Documents (CC&R’S).

I agree to notify ARC upon completion of this project. I understand that the office may conduct a follow-up inspection to ensure completed project are in compliance with request as approved.

SAMPLE SUBMISSION: You MUST provide the ARC Committee with samples of materials or photos to be utilized, photos of the area being changed, map showing the location of the project on the property and any set of plans depicting the location.

ALL PAINTING REQUESTS REQUIRE SAMPLES TO BE PAINTED (12" X 12") ON THE EXTERIOR OF THE HOUSE FOR THE ARC COMMITTEE TO VISIBLY SEE FROM THE STREET.
